Short-form Surveys
With guidance from sector-specific committees, the OHA has created short-form patient experience surveying instruments to meet the needs of hospital staff and patients for surveys shorter in length than traditional long-form surveys. Short-form surveys are compatible for use with electronic platforms (e.g., email, SMS/text) and enable hospitals to obtain more patient experience feedback faster using surveying modes that are cost-effective.
Like long-form surveys, the results from short-form surveying tools can be used to inform inter- and intra-hospital comparison, benchmarking, and quality improvement initiatives. The short-form surveys will be available once a new contract agreement for patient experience measurement services is established for the 2022-23 fiscal year.
Guiding Principles for Short-Form Survey Selection
Since 2019, the OHA has led survey review committees made up of hospital and system stakeholders including representatives from hospitals, physicians, patients, Health Quality Ontario, and Local Health Integration Networks, to develop short-form survey tools for use by Ontario hospitals. Currently, short-form surveys have been finalized for the Adult Inpatient, Pediatric Inpatient, and Adult Emergency Department (ED) sectors. The development of surveys for additional sectors is planned for the near future including the Outpatient, Day Surgery, Pediatric ED, and Mental Health sectors.
Available Surveys
Below, you will find links to the short-form patient experience survey instruments recommended by the OHA-led survey review committees. Hospitals can view the complete list of the pre-programmed 20+ standardized patient experience surveying instruments the OHA has made available in the Qualtrics XM platform including proprietary and non-proprietary surveys. This guide has been updated to clarify what survey modes are available and instructions for obtaining permissions to use any of the five proprietary surveys.
Please note: Under the approval of the Emergency Services Advisory (ESAC) Committee, hospitals can use either the Ontario Emergency Department Short-form (OED-SF) Patient Experience Survey or Ontario Emergency Department Patient Experience of Care (EDPEC) survey to participate in Ontario's Pay for Results Program (P4R).
