Often recognized as the “front door” to health care, the teams in our Department of Family and Community Medicine play a critical role in caring for patients across the lifespan, building longitudinal relationships and promoting health within our community.
Dr. Daniel Lindsay and Dr. Duaa Yahya are completing their Family and Community Medicine residency at Sunnybrook as part of the hospital’s Integrated Community Program with the Academic Family Health Team. As part of their training, they see patients at all stages of life. From prenatal care to newborn visits, all the way to home visits for palliative care, family physicians and residents are essential for preventative health, chronic disease management and overall wellbeing for their patients.
At Sunnybrook, learners in the Family and Community Medicine Residency Program have an opportunity to work closely with an interprofessional team, including nurses, pharmacists, dietitians, social workers and more. Unlike other training programs, Sunnybrook Family and Community Medicine residents also have access to work with physicians across our 11 specialized care programs, and broaden their impact by working with our community care partners like Vibrant Community Health, North Toronto Primary Care Network and Sprint Senior Care.
