One of the notable patient, family and caregiver-driven advancements is the redesign of standardized white boards in hospital rooms. These white boards serve as a vital collaborative communication tool, allowing health care providers to share important information with patients and their families. The boards include details such as daily schedules, medication lists, and care plans, ensuring that everyone is involved and engaged. This initiative has significantly improved communication, coordination of care, fostering a more inclusive and supportive environment for our patients, families and caregivers. Another program at OSMH that enhances patient experience is the Essential Care Provider (ECP) initiative. Recognizing the crucial role that family members and caregivers play in a patient's journey, OSMH developed a program that allows designated individuals to be actively involved in the care pathway. Upon application and approval, ECPs work with the health care team to provide emotional, physical and social support to assist with daily activities. This program not only strengthens the bond between patients and their loved ones but also enhances the overall quality and safety of care provided.
OSMH's commitment to patient-centered care is further exemplified through our patient experience surveys. These surveys are designed to gather feedback from patients, families, and caregivers which in turn allows OSMH to continuously improve our care. Together with our patients, families and caregivers, we created program-specific surveys. The insights gained are invaluable in identifying areas of strength and opportunities for growth. Through our commitment to patient surveys, OSMH ensures that patient experience remains at the forefront of decision-making processes.
In addition to these initiatives, OSMH has implemented a new patient-focused policy: 'Animals at OSMH.' This policy recognizes and respects the importance of service animal at OSMH. Co-created with patients and supported by numerous committees, including: Accessibility, IPAC, Mental Health, Occupational Health and Safety and Quality, this important document reflects OSMH's commitment to inclusive and accessible care.
At the heart of OSMH's success is the incredible dedication of the Patient and Family Advisory Council (PFAC). This council comprises patients, family members, and caregivers who work closely with leaders to provide valuable insights and recommendations. The PFAC plays a pivotal leadership role in shaping policies, programs, initiatives, strategy and ensuring that the patient perspective remains at the core of all our programs.
