Navigating Ontario's complex health care system is not a simple task. Clinical patient navigators play a pivotal yet challenging role in guiding patients through the entire health care system, allowing for a seamless journey along the continuum of care. This role is vital as Ontario transforms to an integrated health system centred around patients, families, and caregivers.
COVID-19 has raised new challenges for those who support patient transition and navigation through hospitals and across the sector. Navigators must now guide patients through a health care system experiencing significant service disruptions, expanding virtual care options, and rapidly changing directives. This is the time to come together for collaborative problem solving and shared learning to support this essential role.
In recognition of this need, the OHA is launching a series of virtual sessions for health care professionals transitioning and navigating patients through the hospital and across the health care sector. Sessions will explore the role of the patient navigator in different settings; discuss challenges of the role especially during this pandemic; and take a deep dive into innovative patient navigation models currently used in Ontario hospitals.
