Digital Health as an Enabler of Integrated Care

In a time of rapid, transformational change, Ontario's health system is faced with an unprecedented opportunity to re-imagine care through digitally-enabled solutions. Advancements in digital health allow us to build a connected system that is patient-centred and promotes a bold new vision of health.

To reframe digital health as an enabler of integrated care, the Ontario Hospital Association in partnership with the Institute for Health System Solutions and Virtual Care (WIHV) at Women's College Hospital have co-designed a one-day program to help us re-think digital transformation for health care.

Digital Health in Ontario: Beyond the Technology

To address the needs of patients and the growing strain on our health care system, Dr. Sacha Bhatia, Chief Medical Innovation Officer at Women's College Hospital sets a bold vision for the future of integrated health care and the role of digital technology. With a focus on integrated care, he discusses the current and future implications of digital health across clinical practice, health care organizations and the broader health system by sharing thoughtful approaches for digital transformation to reshape health care for Ontarians.


Leveraging Virtual Care Models to Deliver Higher Value Care
Dr. Ed Brown, CEO at the Ontario Telemedicine Network (OTN) looks at the virtual care advancements taking place in Ontario, the experiences of organizations that have successfully introduced technology-enabled models of care, and how others can draw on lessons learned and plan to meet their own clinical care goals. The session will provide valuable insight to those new to virtual care and those seeking to expand their use, focusing on critical success factors like patient population prioritization, data, virtual care opportunities and solutions, common pitfalls and effective change management. Prepare to be inspired by the art of the possible and gain an understanding of how to move toward implementation, in order to improve the patient experience and care delivery processes, while also realizing efficiencies.

Digital Transformation and Service Redesign: Building Blocks for Primary and Integrated Care
Integrating care involves aligning the interests of different stakeholders and finding a common value proposition for digitally enabled services. During this session, Dr. Onil Bhattacharyya, Frigon-Blau Chair in Family Medicine Research at Women’s College Hospital  will highlight the potential of tools like virtual visits, care coordination platforms and remote monitoring systems to improve access, coordination and continuity of care in an integrated care delivery system. This session will also introduce methods for service redesign using digital tools through a series of case studies from primary care and community hospitals in Ontario, and showcase practical approaches to starting the digital transformation of health care.


Building the Virtual Hospital: Revolutionizing Care Through Design
The future of technology-enabled care is on the horizon. Dr. Danielle Martin, Executive Vice President and Chief Medical Executive Women's College Hospital   will share how Women's College Hospital is creating Canada's first virtual hospital, revolutionizing how care is delivered in Ontario.
