Upcoming OHA Conference: Family Health Team Implementation: Challenges, Barriers and Opportunities

For the Attention of Hospital CEOs, OHA Board of Directors and CACN Members

From Warren DiClemente, Vice President, Educational Services

As you know, the Ontario Hospital Association (OHA) offers a number of educational conferences each year that are designed by and for our members, and aim to share best practices and information about industry-wide trends and issues that are important to hospitals.

As part of its commitment to highlight relevant issues in health care, the OHA in conjunction with the Ontario College of Family Physicians, Ontario Association of Community Care Access Centres, and the Ontario Pharmacists’ Association is holding a conference on Family Health Team Implementation: Challenges, Barriers and Opportunities, which should be of interest to members.

A key priority of the government's health care agenda is to deliver to Ontarians the very best access to primary care. Family Health Teams (FHTs) will be comprised of doctors, nurse practitioners, nurses, and other complementary medical practitioners working together to meet patients' individual needs around the clock, seven days a week. The teams will emphasize prevention and healthy living as much as treating illness.

Seventeen Family Health Teams are in the process of being established in South Western Ontario. Leaders in the industry will share their insight and discuss topics including:
• Family Health Teams Update from Dr. Jim MacLean, Lead, Primary Care Reform, MOHLTC
• Family Health Team Action Group Update
• How to Access Free Information Technology Services
• Governance Models: Legal Issues and Barriers
• Collaborative Family Health Teams - Chronic Disease Management
• LHINs and Primary Care Development from Paul Huras, LHIN CEO, South Eastern Ontario
