2008 Patient Safety Guidebook to Leading Patient Safety Practices in Ontario Hospitals



Following the success of the 2006 publication “Inspiring Ideas and Celebrating Successes: A Guidebook to Leading Patient Safety Practices in Ontario Hospitals” the OHA is pleased to commence the development of “The 2008 Patient Safety Guidebook to Leading Patient Safety Practices in Ontario Hospitals.”

As was the case for the 2006 publication, the 2008 Patient Safety Guidebook will also be a vehicle to display and share innovative initiatives from hospitals across the province. The purpose is to provide a “sampling” of practical and effective patient safety strategies that other hospitals can explore, learn from and potentially initiate within their own facility. The 2008 Guidebook will be organized around the six CCHSA patient safety areas: Culture; Communication; Medication Use; Worklife/Workforce; Infection Control; and Falls and will highlight the hospitals that have introduced patient safety practices and strategies in one or more of these areas.

The release of the booklet is scheduled for October 2008 to coincide with Canadian Patient Safety Week.

Submission Guidelines for the Guidebook:

    Submissions should be no longer than 500 words including:
    · a description of the initiative;
    · its relevance to patient safety;
    · its association to the CCHSA patient safety areas and/or Required Organizational Practices (please clearly identify which patient safety area and, if applicable, which ROP the initiative is aligned with);
    · practical implications and key success factors for implementation;
    · a description of how processes/systems or patient outcomes have improved (including outcome measures/evaluation if available); and
    · the cost effectiveness of the strategy (if applicable)

    Submissions will be selected for inclusion in the booklet based on the following criteria:
    · Originality/Innovation
    · Broad Applicability
    · Practicality/Relevance
    · Association to CCHSA patient safety areas and/or ROPs
    · Demonstrated outcomes

    Submissions are due March 28, 2008. All successful submissions will be notified in June 2008.

2008 Patient Safety Award


The purpose of the award is to acknowledge exemplary work in the area of patient safety at the hospital level and to promote effective strategies that enhance patient safety. The first Patient Safety Award was presented at OHA HealthAchieve 2005 to the Hospital for Sick Children for their Checklist for Intra-Hospital Patient Transfers.

Based on the submissions received for the Guidebook, a single initiative will be chosen with a focus on innovation and demonstrated outcomes in patient safety practice or processes. The recipient of the award will be highlighted in the Guidebook and may be asked to expand their submission.

The award recipient will be notified in June 2008.

The 2008 Patient Safety Award will be presented at OHAHealthAchieve 2008.


2006 Patient Safety Booklet


The booklet entitled, Inspiring Ideas and Celebrating Successes: A Guidebook to Leading Patient Safety Practices in Ontario Hospitals is a vehicle to display and share innovative initiatives from hospitals across the province. The purpose is to provide a “sampling” of practical, effective patient safety strategies that other hospitals can explore, learn from and potentially initiate within their own facility.

The release of the booklet coincides with the 2006 Canadian Patient Safety Week (October 20-27).
