Your Health Care - Be Involved (Home and Community Sector) 

 

“Your Health Care – Be Involved” for the home and community sector is an initiative developed and managed in partnership by the Ontario Hospital Association (OHA) and the Ontario Association of Community Care Access Centres (OACCAC).  Originally launched for hospitals in 2005, the “Your Health Care – Be Involved” campaign has been re-launched, with a targeted focus, this time, on the home and community care settings.  The campaign has been re-branded and re-designed and the five common-sense “tips” updated to better meet the needs of the home and community sector.

 

“Your Health Care – Be Involved” for the home and community sector is a province-wide campaign designed to ensure clients have the information they need to make the most of every health care experience.  The client safety tips campaign is designed to aid in the transitions clients make as they move through the continuum of care.  It is guided by the concept of empowering clients to make informed decisions about their health care, positively transforming the culture of client safety.
