Introducing the Professional Staff Credentialing Toolkit  (#588)

Board-appointed Professional Staff have a direct impact
on the quality of care provided in a hospital, and for that
reason, there must be an effective method to ensure that
hospitals’ credentialling process is sound. To support hospitals with their relationships with Professional Staff, the Ontario Hospital Association (OHA) and its Governance Centre of Excellence (GCE) are pleased to
announce the release the Professional Staff Credentialing
Toolkit.
This webcast will give participants an understanding of why it is critical for hospitals to commit to robust credentialing processes that clarify expectations, enhance
transparency, and contribute to positive working environments. The webcast will provide an overview of key
sections of the Toolkit and of the many practical sample
templates it contains. Participants will have an opportunity
to ask questions about how the Toolkit may impact their
hospital’s credentialing practices.

Patient Safety and Narcotic Administration- Lessons Learned from the Coroner’s Office  (#587)

The Canadian Adverse Events Study, published in 2004,
found that 7.5% of people admitted to hospitals in Canada
experienced at least one adverse event and that almost 21% of such adverse events were fatal. The Office of the Chief Coroner for Ontario serves the living through high quality death investigations and inquests to ensure that no death will be overlooked, concealed or ignored. The findings are used to generate recommendations to help improve public safety and prevent deaths in similar circumstances.

The Patient Safety Review Committee (PSRC) assists
the Office of the Chief Coroner in the investigation,
review and development of recommendations towards
the prevention of deaths regarding health care-related
cases where system-based errors appear to be a major
factor. The PSRC also assists coroners in improving the
investigation of deaths within, or arising from, the health
care system in which system-based errors appear to have
occurred.

This webcast features the PSRC findings and
recommendations on narcotic medication use in hospitals.
Because of the frequency of their use, it is easy for
providers to lose sight of the fact that these medications
carry with them significant, and even fatal, side effects.
This session is case-based, and focused on common
sources of narcotic medication error, from prescribing
to dispensing to administration to monitoring. Through
exploration of these cases, the participant will gain an
understanding of the underlying contributing actors and
system-based strategies to prevent narcotic medication
errors and serious side effects of narcotic administration.

Building a High Performing Board: Best Practices in Director Recruitment (#586)

The Governance Centre of Excellence (GCE) is pleased
to present the webcast “Building a High-Performing
Board: Best Practices in Director Recruitment”. Director
recruitment is often event driven: the annual meeting is
fast approaching; there are vacancies on the board that
need to be filled; quick consideration is given to who can
quickly be tapped into serving as a Director or who owes
the organization a favour. It’s not surprising that many
boards don’t feel they’re ideally equipped to fulfill their
responsibilities. Working within whatever restrictions may
be required by such things as the organization’s by-laws,
boards should consider undertaking an effective process
to help ensure an optimal mix of skills and experience.

An effective board recruitment and nominations process
is essential for bringing the best talent to the board.
This session will provide strategies and resources
for recruiting board directors that possess the skills,
knowledge and other attributes needed in order for the
board to effectively carry out its responsibilities.
Participants will receive a clear understanding of the core
competencies required for the position, the role of the
Board Chair and Nominations Committee and elements
of an effective recruitment and orientation process.
