Registration
Registration will take place on Monday, September 12, 2011, at 8:30am at the Sheraton Toronto Airport Hotel & Conference Centre, 801 Dixon Road, Toronto.
Space is not guaranteed, unless payment is received prior to the event.
Registration Fee
Member (OHA/OHPA/MOHLTC):
$890.00 + HST $115.70 = Total $1005.70
Non-member:
$1200.00 + HST $156.00 = Total $1356.00
Discount Policy
If more than one full registration is received from the same facility, a 15% discount (+13% HST) will apply to each additional registration received. The first registrant must pay the full registration fee. All group registrations must be received at the same time. This discount is not valid with other program promotions.
Cancellation Policy
A $200.00 processing fee per registrant will apply to cancellation refunds received in writing up to five business days prior to the event. No refunds will be given for cancellations received less than five business days prior to the event. Substitutions are welcome. The OHA reserves the right to cancel or reschedule
an event.
Accommodation
If you require accommodations, please contact the Sheraton Toronto Airport Hotel & Conference Centre at 1-800-325-3535. Please be sure to indicate that you are attending the OHA conference to secure a special room rate of $174 for single/double occupancy. Be sure to make your hotel reservation before August 9, 2011, as accommodations for that time period are filling up quickly.
If you require extra assistance or have special requirements, please let us know when registering for the conference to ensure we can accommodate them.
