The Ontario Hospital Association (OHA) and the Ontario Association of Community Care Access Centres (OACCAC) have developed a four-part webcast series addressing the Alternate Level of Care (ALC) challenges and the different stages of the transition to care. This webcast is the first of the series and will focus on provisions under the Health Insurance Act, the Long-Term Care Homes Act and Regulation 79/10 as well as the Public Hospitals Act that must be considered when transitioning patients after their acute stay. The webcast will also profile one hospital’s/CCAC’s initiative for transitioning patients to LTC that has come to be accepted as a leading practice.
