Complex Continuing Care (CCC) and Rehabilitation (Rehab) services care for very ill and medically-complex patients. These services help facilitate the improvement of patient flow through our healthcare system and enhance the recovery process of patients who later return home or into their communities.
The Complex Continuing Care and Rehabilitation Provincial Leadership Council works closely with the OHA to provide a broader understanding about the potential role the provinces CCC and Rehab providers play in integrating care to help alleviate systemic pressures while contributing to changes in patterns of practice.
Membership of the Council consists of CEO representatives from specialty complex continuing care and rehabilitation hospitals and acute hospitals with complex continuing care and rehabilitation beds/services.
